First Class Mobility Inc. provides non-emergency medical transportation in Illinois. This policy explains how we collect, use, and share information through our website and when arranging or providing transportation. It applies to First Class Mobility Inc. and does not cover separately operated businesses.
1. Information we collect
We collect information you or an authorized person provide when requesting a quote, booking transportation, contacting us, or paying for a ride. This may include the rider’s name, contact information, pickup and destination addresses, appointment and trip times, emergency or authorized contact details, and transportation assistance needs, such as wheelchair or stretcher accommodation.
We may receive information from family members, caregivers, healthcare facilities, health plans, or transportation brokers involved in arranging a ride. For covered transportation, this may include member identification and information needed to verify eligibility, coordinate the trip, and submit payment requests. We also maintain trip, communication, consent, and payment records.
Please provide only information needed to arrange transportation. Do not send medical records, diagnoses, Social Security numbers, or payment card details through ordinary text messages or general website inquiries.
2. Website information and cookies
Our website uses Google Analytics and Google Ads measurement tools, managed through Google Tag Manager, to understand website activity and measure advertising results. These tools may collect browser and device information, cookie or similar identifiers, pages viewed, referring websites, approximate location, and actions such as selecting a booking link. Our hosting provider, Cloudflare, may process IP addresses, requested pages, and other technical logs for website delivery and security.
Information collected by these tools is not necessarily anonymous. Google’s processing is also described in its privacy policy at policies.google.com/privacy. We do not authorize the use of rider medical information or detailed trip records for advertising.
You can block or delete cookies in your browser. Google provides an Analytics opt-out browser add-on at tools.google.com/dlpage/gaoptout and advertising controls at myadcenter.google.com. These controls do not necessarily stop every type of website data collection. Our website does not currently respond to browser Do Not Track signals. Where applicable law requires consent or recognition of another privacy preference signal, we will comply with those requirements.
3. How we use information
We use information to respond to inquiries, provide quotes, schedule and perform rides, arrange appropriate vehicles and assistance, communicate trip updates, coordinate with authorized contacts and facilities, process payments, and handle questions or complaints. We also use relevant information to maintain business records, protect safety and security, prevent misuse, and satisfy applicable legal and contractual obligations.
Website usage information helps us improve the website and measure advertising performance. Rider health information and detailed trip records are used for transportation and related administrative purposes, rather than advertising.
4. Booking platforms and trip location
Our online booking link connects to Duet, operated by Dashboard Story, Inc. Duet supports reservations, dispatch, driver services, trip records, tracking links, and reminders. Information submitted through that platform is processed in connection with our transportation services. Duet’s privacy information is available at duetinc.com/legal.
Trip location and timing information may be generated through the driver application and vehicle movements. Tracking links may reveal trip status or location to their recipients. Keep those links private and share them only with people authorized to receive that information.
5. How we share information
We share relevant information with personnel and service providers who support transportation, scheduling, communications, payments, website operation, and business administration. Providers may include Duet, payment processors, hosting providers, communications services, and Google for the website analytics and advertising measurement described above.
We may share trip information with healthcare facilities, health plans, transportation brokers, and contacts authorized by the rider or otherwise permitted by law. We limit disclosures to information relevant to their role. A person’s arrangement of a ride does not, by itself, authorize unrestricted access to the rider’s information.
We may disclose information when required or permitted by applicable law, to address safety or security concerns, or to establish or defend legal rights. Information may be transferred as part of a proposed or completed business transaction, subject to applicable confidentiality requirements and lawful limits on further use.
We do not sell rider contact information, health information, or detailed trip records. We do not provide those records to third parties for their independent marketing. Website analytics and advertising measurement are described separately above.
6. Health-related information
Transportation details may reveal health-related information, including assistance needs or travel to a healthcare facility. We treat this information as confidential and limit its use and disclosure to transportation, related administration, and other purposes permitted by applicable law.
Where HIPAA applies to our activities, we handle protected health information in accordance with the requirements applicable to our role. This website privacy policy does not replace any legally required Notice of Privacy Practices or limit rights under applicable health privacy laws. Contact us for information about the privacy rights and notices applicable to your records.
7. Text messages and other communications
With your consent, we may send transportation-related text messages, including quotes, confirmations, reminders, tracking links, and trip updates. Message frequency varies. Message and data rates may apply. Consent to receive texts is not a condition of purchasing transportation.
Reply STOP to opt out or HELP for assistance. You may also contact us at 331-313-9917 to withdraw consent or request another communication method. Opting out of texts does not cancel an existing ride; please contact us separately to change or cancel a reservation. Carriers are not liable for delayed or undelivered messages.
We do not share mobile information with third parties or affiliates for marketing or promotional purposes. Text messaging originator opt-in data and consent are not shared with third parties, except aggregators and providers of the text messaging services needed to deliver our messages.
Ordinary texts and email may be viewed by someone with access to your device or account. Avoid sending sensitive information through these channels. If we record or transcribe calls, we provide notice and obtain consent as required by applicable law before doing so.
8. Payments
Payment processors, banks, or payment platforms handle electronic payments under their own privacy and security terms. We maintain information needed to confirm payment, such as the payer’s name, amount, date, transaction reference, and payment status. Please use the designated payment method rather than sending card numbers or banking credentials by text or email.
9. Retention and security
We retain information for as long as reasonably necessary for the purposes described in this policy, including transportation, billing, required recordkeeping, claims, audits, and dispute resolution. Retention depends on the record type and applicable legal and contractual requirements. Information subject to a legal hold may be retained longer. When information is no longer needed, we arrange for its deletion, destruction, or de-identification, subject to applicable requirements and backup retention cycles.
We use reasonable administrative, technical, and physical safeguards appropriate to the information we handle. No method of storage or transmission is completely secure. If an incident requires notification, we will provide notices in accordance with applicable law.
10. Your choices and privacy requests
You may contact us to request access to, correction of, or deletion of personal information we maintain. We will verify your identity and, where relevant, the authority of anyone acting for you. We address requests in accordance with applicable law and may retain information needed for legal, contractual, billing, safety, or dispute-related purposes. Some records may be maintained by a healthcare facility, health plan, or broker; we will explain when a request needs to be directed to another organization.
Available rights and response requirements depend on the laws applicable to the information and our activities. We will not retaliate against you for exercising a legally protected privacy right. You may withdraw text-message consent as described above.
11. Children and other websites
Our website is intended for adults arranging transportation. We do not knowingly collect information directly online from children under 13. A parent, guardian, or other appropriately authorized person should arrange transportation for a minor. Contact us if you believe a child has submitted information directly through the website.
Independent websites linked from our site have their own privacy practices. Where a linked service processes information on our behalf, our applicable responsibilities continue to apply.
12. Changes to this policy
We may update this policy to reflect changes in our services, information practices, or legal requirements. We will post the revised policy with an updated effective date and provide additional notice or obtain consent where required by applicable law.
